Waivers

Errors, Omissions, and Negligence waiver

I hereby release Ewens Toxicology Consulting, LLC and Dr. Andrew Ewens, PhD, DABT (Dr. Ewens), bound by the laws of the state of North Carolina in the United States of America, from any and all claims of harm, injury, or damage due to errors, omissions, or negligence.  I understand that interpretations do not constitute a diagnosis; are subjective, because they involve assumptions, professional judgment, and factors outside of Dr. Ewens’ control; and that other experts may have a different interpretation of the same test or report.  I understand that beneficial outcomes on my or my client’s case are not guaranteed.

Loss of Privacy waiver

I hereby release Ewens Toxicology Consulting, LLC and Dr. Andrew Ewens, PhD, DABT (Dr. Ewens), bound by the laws of the state of North Carolina in the United States of America, from any and all claims of harm, injury, or damage due to the malicious actions of persons other than Dr. Ewens or due to accidental actions and mistakes.  I agree to the privacy policy which states the following:

Privacy Policy

Medical, legal, or forensic information with personally identifying information will be kept confidential and will not be released to persons outside of Ewens Toxicology Consulting, LLC, except when required for my case. Medical, legal, or forensic information without associated personally identifying information may be used in an anonymous manner for purposes other than forming an expert opinion about my situation.  Such purposes may include the publication of case reports in any form, educational purposes, promotions of Ewens Toxicology Consulting, LLC, or other purposes that do not identify me or anyone involved in my situation. Personally identifying information may be used for purposes other than forming an expert opinion about my situation to allow for continued contact for the purposes of delivering newsletters, payment disputes, advertisements, or other correspondence with Ewens Toxicology Consulting, LLC.
